No referral is required. While we often receive referrals from home health agencies, patients and caregivers may also contact us directly to schedule a visit.
Yes. If you do not currently have a primary care provider—or cannot access yours regularly, we can serve as your ongoing provider for routine and chronic care management.

We provide weekly visits until the wound is healed, including assessments, cleaning, treatment, and patient education.
Our medical assistants come to your home to collect blood, urine, or sputum samples. Once labs are processed, your nurse practitioner will review the results with you and update your care plan as needed.
Yes. We provide in-home X-rays and weekly ultrasound appointments. Imaging is coordinated by our team and reviewed as part of your care plan.
We offer prescription management for chronic conditions as well as short-term refills after hospital discharge. Your provider will review your medications during visits and coordinate with your pharmacy to ensure continuity of care.
We can provide interim care if you’re unable to get an appointment right away, for example, after a hospital stay. Our team can manage your medications and monitor your condition until you’re able to reconnect with your primary provider.
